All people attending an event sponsored or organized by Knox Church during the time of COVID-19 restrictions must complete and submit this form at the bottom of this page in advance of the event. By doing so, you agree that you will observe safety practices as directed by the State of Ohio and local governments and health authorities. This includes the wearing of a mask at all times and keeping a distance of six feet from other individuals and/or family groups. You understand that if you do not observe these practices you may be asked to leave the event.

ONLY ONE COMPLETED FORM PER HOUSEHOLD PER EVENT IS NEEDED. FOR CHILDREN AND YOUTH UNDER THE AGE OF 13, THE FORM MUST BE COMPLETED BY A PARENT OR GUARDIAN.
THIS FORM MUST BE COMPLETED EACH TIME YOU ATTEND ANY KNOX-SPONSORED EVENT. (FOR EXAMPLE, COMPLETE THE FORM IN ADVANCE OF EVERY TIME YOU ATTEND CHOIR PRACTICE.)
FORMS MUST BE COMPLETED WITHIN 72 HOURS OF THE START OF THE EVENT.
IF YOU OR A MEMBER OF YOUR HOUSEHOLD BEGINS TO EXPERIENCE SYMPTOMS OF CORONA VIRUS WITHIN TWO WEEKS OF ATTENDING A KNOX EVENT, PLEASE NOTIFY A KNOX PASTOR.
IF YOU ANSWER 'YES' TO EITHER OF THE FIRST TWO QUESTIONS, PLEASE DO NOT ATTEND THE EVENT.
